BRAZILIAN MONTHLY WAX MEMBERSHIP AGREEMENT                                              

                               

Month-to-Month Billing 

 

This membership agreement between Feather and Companies LLC dba Exquisite Esthetics and  (Member Name) shall be effective on the date the payment plan is purchased online. Benefits will become available after the above billing date each month. 

 

        MEMBERSHIP INCLUDES: One Monthly Brazilian Wax Only.  The Brazilian Wax must be booked consecutively month to month for a total of 1 Brazilian Wax per month. Included and discounted treatments may be used only by the member. 

Membership: 

1. This Agreement is personal to the member and may not be assigned, transferred or otherwise disposed of by the member.

2. Applicants for membership must be at least 18 years old. Applicants under the age of 18 years old must have parental consent.

 

Automatic Payment Agreement:

1. Weeky membership payments shall be made in advance by direct debit from the member’s designated Paypal account. Either party may cancel the membership at any time by giving a 30 day written notice to the other party. 

2. Feather and Companies LLC dba Exquisite Esthetics reserves the right to review subscriptions periodically. Members will be given at least a 30 days notice in writing of any changes, which include: (i) any increase in membership fee, (ii) change in date of automatic withdrawal. 

 

Termination or Suspension of Membership: 

Feather and Companies LLC dba Exquisite Esthetics reserves the right at any time to cancel or suspend the membership of any member in the event of the following: 

• The member commits a serious breach of this Agreement and/or Feather and Companies LLC dba Exquisite Esthetics Rules and Regulations.

• Where any monies are due to Feather and Companies LLC dba Exquisite Esthetics by the member remain unpaid for 30 days after its due date for payment.

• The member knowingly provides false details when applying for membership and the false declaration would have reasonably affected Feather and Companies LLC dba Exquisite Esthetics decision to grant the membership. 

• If Feather and Companies LLc dba Exquisite Esthetics terminates for any reason, they reserve the right to retain any monies received to cover any reasonable costs they have incurred as a result. 

 

Terms and Conditions: 

1. Feather and Companies LLC dba Exquisite Esthetics reserves the right to vary, add or eliminate any of the particular services and facilities provided by them from time to time. 

2. Feather and Companies LLC dba Exquisite Esthetics reserves the right to set aside facilities for social events or activities.

3. Feather and Companies LLC dba Exquisite Esthetics reserves the right to close or modify facility hours with or without notice.

4. May not be combined with any other promotional offer, discount, coupon or promotional gift card.

VOLUME MONTHLY FILL + SKIN CARE FACIAL MEMBERSHIP AGREEMENT                                              

                                 

Month-to-Month Billing 

 

This membership agreement between Feather and Companies LLC dba Exquisite Esthetics and (Member Name) shall be effective on the date the payment plan is purchased online. Benefits will become available after the above billing date each month. 

 

        MEMBERSHIP INCLUDES: This option is for Clients who are signed up for monthly auto payments for the VOLUME MONTHLY FILL + a Skin Care Facial Package. The one 1 Hour Fill and 1 Hour Facial must be booked on the same day for a total of 2 hours for the appointment and consecutively month to month for a total of 12 Fills. *Additional Fills during the same month must be booked online at the regular price with paid deposit. Included and discounted treatments may be used only by the member.

MEGA VOLUME WEEKY FILL MEMBERSHIP AGREEMENT                                              

                                 

Week-to-Week Billing 

 

This membership agreement between Feather and Companies LLC dba Exquisite Esthetics and  (Member Name) shall be effective on the date the payment plan is purchased online. Benefits will become available after the above billing date each month. 

 

        MEMBERSHIP INCLUDES: One 1 Hour MEGA VOLUME WEEKLY FILL ONLY.  The one 1 Hour Fill must be booked consecutively week to week for a total of 1 Fill per week. Included and discounted treatments may be used only by the member. *Additional Fills during the same month must be booked online at the regular price with paid deposit.

 MONTHLY DERMAPLANE + CHEMICAL PEEL MEMBERSHIP AGREEMENT                                              

                                 

Month-to-Month Billing 

 

This membership agreement between Feather and Companies LLC dba Exquisite Esthetics and (Member Name) shall be effective on the date the payment plan is purchased online. Benefits will become available after the above billing date each month. 

 

        MEMBERSHIP INCLUDES: This membership is for one MONTHLY DERMAPLANE + one GLYCOLIC CHEMICAL PEEL. This is a 75 minute appointment, consecutively month to month for a total of 12 appointments. Cannot split services for different times or days.

THE MANDY PLAN MEMBERSHIP AGREEMENT                                              

                                 

Month-to-Month Billing 

 

This membership agreement between Feather and Companies LLC dba Exquisite Esthetics and  (Member Name) shall be effective on the date the payment plan is purchased online. Benefits will become available after the above billing date each month. 

 

        MEMBERSHIP INCLUDES:  1 session per month of the following - 1 Brazilian Wax, 1 Skin Care Facial, 1 60 Minute Classic Lash Fill.  Package/Plan appointment must be booked online and used on same day in a 3 hour time slot.  Services cannot be broke up to be used on other days during the month. Included and discounted treatments may be used only by the member.
